Changes to M-186 form to protect your medical privacy

To: All MoveUP members at Coast Mountain Bus Company


Your employer has informed us that the M-186 form will be modified as of April 14, 2025.

This comes after a policy grievance we brought forward on behalf of all members regarding the collection of medical information on the M-186 form.

This change is a victory for us and better secures your private medical information.

What changes?

The M-186 form will no longer require the disclosure of diagnoses, and it will no longer ask for an employee to consent to direct communication between Occupational Health and the employee’s physician.

The new form will ask your doctor to describe the nature of your illness. Please note that your doctor should not provide your specific diagnoses on this form. This is private medical information that cannot be unshared.

If you are asked by an agent of the employer to provide your diagnoses for any reason, please reach out to a job steward or union representative immediately.

Next steps

We are still in the process of determining if the change to the form is enough to resolve our policy grievance. We will provide further updates as they become available.
