TransLink – COVID-19 Form – Update #3 – March 20, 2020

To: All MoveUP members at TransLink
Due to the unprecedented circumstances the Employer is facing and in consultation with your Union, effective immediately, the M-186 form will not be required for employees to complete in cases related to COVID-19 or potential COVID-19 like cases (i.e., respiratory symptoms such as cough, fever, difficulty breathing etc.). The M-186 form will be replaced with the attached “Confirmation of Illness Form”. This process is being put in place to limit the impact on the health care system and to prevent unnecessary contact that may further spread the virus. Your Union is still advocating to the Employer to suspend the Attendance Management Program during this unprecedented time.
The Confirmation of Illness Form should be completed by employees who are exhibiting COVID-19 or COVID-19 like respiratory symptoms (e.g., cough, fever, difficulty breathing etc.) and a copy provided to Occupational Health by email at OccupationalHealthGroup@Translink.ca. A physician or medical professional does not need to complete the form.
Please note that employees who are going to be absent due to medically advised self-isolation (doctor, health authority or BC Health Link line 8-1-1 advisor), self-isolating after returning from overseas travel, or have COVID-19 symptoms should still contact their manager to advise of their absence as well as Occupational Health and their health care provider for guidance.
